The Frequency of Odontogenic Cysts and Tumors and Their Association with Impacted Teeth

Background and Objective: Odontogenic cysts and tumors are common lesions in the jaws that association with impacted tooth can cause serious problems. The present study aimed to investigate the frequency of odontogenic cysts and tumors and their association with impacted teeth over an 11-year period.
Methods: In this retrospective descriptive study, all records from the Oral Pathology Department of Babol Dental School from 2012 to 2022 were reviewed. Demographic characteristics of the patients, lesion location, associated impacted teeth, clinical and radiographic features, clinical and/or radiologic differential diagnosis, histopathological diagnosis, biopsy type, and macroscopic features of the lesions were recorded.
Findings: Of the 1512 samples, 428 (28.3%) were diagnosed as odontogenic lesions (85% cysts and 15% tumors). Most patients (54.4%) were male. The mean age of patients with cysts was 33.02±15.31 and with tumors was 32.53±16.08 (p=0.819). Most lesions (27.1%) were in the third decade of life and (92.3%) had a radiolucent appearance. 75% of odontogenic tumors and 61.8% of odontogenic cysts were found in the mandible (p=0.043). The most common odontogenic cysts were radicular cyst, dentigerous cyst, and odontogenic keratocyst. The most common odontogenic tumors were ameloblastoma and complex odontoma. The most common lesion in the patients under 20 years was dentigerous cyst and 21 years and older was radicular cyst. 31.5% of the lesions were associated with impacted teeth, and the most common impacted tooth was the third molar (58.2%). Clinical and radiological diagnosis of 292 cases (68.2%) was consistent with histopathological diagnosis.
Conclusion: According to the present study, the frequency of odontogenic lesions was higher than other studies, and a significant association of these lesions with impacted teeth was observed. Therefore, the presence of impacted teeth can be considered as a potential risk factor for the presence of odontogenic lesions. |
