Relationship between neovascularization of ovarian endometriomas measured by transvaginal Doppler ultrasound and pelvic pain severity

Introduction: Pelvic pain and dysmenorrhea are main symptoms of endometriosis. There is evidence that high vascularity of endometriomas is associated with more severe pelvic pain, but this has not proved to date. The aim of this study was to investigate the relationship between neovascularization of ovarian endometrioma on vaginal Doppler ultrasound with pelvic pain severity.
Methods: In this cross-sectional study, 76 women with ovarian endometrioma (histologically confirmed) were divided into two groups based on pelvic pain severity (using visual analog scale): mild/moderate pain (VAS of 0-66) and severe pain (VAS of 66-100).  All patients underwent vaginal Doppler ultrasound and endometrioma neovascularization as well as resistive index (RI) and pulsatility index (PI) of the endometrial cyst wall artery were measured.
Results: Moderate/severe vascularization severe pelvic pain (50%) was more common in comparison to mild/moderate pelvic pain 913%); P= 0.005. Mean (±SD) RI was significantly lower in severe pelvic pain group (0.59± 0.05) vs. mild/moderate pelvic pain group (0.66± 0.06): P< 0.001.  Mean (±SD) PI in severe pelvic pain group (1.07± 0.13) was significantly lower than that of mild/moderate pelvic pain group (1.14± 0.15); P= 0.03.
Conclusion: High vascularization and lower PI and RI of endometriomas using trans-vaginal Doppler ultrasound reflect endometrioma activity and are associated with pelvic pain.
