Evaluation of the Safety and Efficacy ERCP in Patients with Common Bile Duct Stones in Different Age Groups

Background and Objective: Endoscopic retrograde cholangiopancreatography (ERCP) is an important tool for the diagnosis and treatment of pancreaticobiliary diseases. Complications associated with ERCP in the elderly due to comorbidities may be more than in non-elderly individuals. Given the lack of studies and the different results in studies, the aim of this study was to compare the results of ERCP in elderly and non-elderly.
Methods: In a cross-sectional study, ERCP findings and demographics of patients with common bile duct stones who underwent ERCP at Rohani Hospital in north of Iran were collected. Demographic information and outcomes in terms of treatment success, complications, and risk factors were evaluated and compared in three age groups (>80 years, 65-79 years, and <65 years).
Findings: The mean age of the patients was 62.02±13.46 years. In this study, 51.6% of the patients were in the age group less than 65 years and 17.4% of them were equal to or more than 80 years old. 5.9% of the performed ERCP procedures. Older age was associated with an increased likelihood of failure in the ERCP procedure. Also, the presence of periampullary diverticula could predict failure of the procedure. In patients aged 80 years and over, acute cholangitis (p=0.004) and periampullary diverticula (p<0.001) were significantly more than other age groups.
Conclusion: The results of this study showed that periampullary diverticula and advanced age, especially age over 80 years, should be considered as risk factors for failure of ERCP.
