Effectiveness of Low and High Dose Radioiodine Treatment for Low and Intermediate-Risk Papillary Thyroid Cancer: A Retrospective Comparative Study

Background and Objective: One of the major challenges in the treatment of thyroid carcinoma is the selection of the appropriate dose of radioactive iodine for ablation of the thyroid remnant after surgery. For low- and intermediate-risk patients, the precise determination of the therapeutic dose is still uncertain. The aim of this study was to evaluate the effect of low versus high doses on the treatment outcomes of this group of patients.
Methods: In this retrospective study, patients were divided into two groups receiving low dose 30 mCi and high dose 100 mCi. Age, gender, physical health status, laboratory markers, findings of whole body scan with iodine-131 and ultrasound before radioablation with iodine and at an interval of 1 year were compared.
Findings: A total of 151 patients were enrolled in the study, of whom 124 (82.1%) were female and 27 (17.9%) were male. Based on the first-year follow-up, 33 (21.9%) of the 151 patients were classified as indeterminate responders (IR) and 118 (78.1%) as excellent responders (ER). There was no significant difference in treatment success between groups, and the prescribed dose had no significant effect on treatment outcomes.
Conclusion: In this study, there was no statistically significant association between low and high doses of radioactive iodine in successful ablation of thyroid carcinoma patients with low and intermediate initial risk.
