fa گزارش یک مورد بهبودی مشکل شخصیتی اختلال شخصیت مرزی پس از ضربه سر روانپزشکی Psychiatry گزارش مورد case report <p dir="RTL" style="text-align:justify;text-justify:inter-ideograph;line-height:16.0pt;"><span style="font-family:times new roman;"><strong><span style="color:blue;"><span style="font-size:10.0pt;">سابقه و هدف:</span></span></strong> <span style="font-size:10.0pt;">اختلال شخصیت مرزی که با تغییرات مداوم خلق، هویت و روابط بین فردی همراه است در دسته </span><span dir="LTR"><span style="font-size:10.0pt;">B</span></span><span style="font-size:10.0pt;"> طبقه بندی اختلالات روانی قرار دارد. علاوه بر این منجر به انجام رفتارهایی مانند سوء مصرف مواد، روابط جنسی تکانه ای و خودکشی می شود که برقراری رابطه با آنها و فرآیند درمان این بیماران را دشوارتر می سازد. در این گزارش یک مورد بهبودی مشکل شخصیتی اختلال شخصیت مرزی پس از ضربه سر معرفی می گردد.</span></span></p> <p dir="RTL" style="text-align:justify;text-justify:inter-ideograph;line-height:16.0pt;"><span style="font-family:times new roman;"><strong><span style="color:blue;"><span style="font-size:10.0pt;">گزارش مورد:</span></span></strong><span style="font-size:10.0pt;"> بیمار پسر 28ساله ای است که دارای ملاک های کامل اختلال شخصیت مرزی شامل علائم شخصیتی وی شامل: مصرف مواد، تکانشی بودن، بی ثباتی خلق، دوره های خشم و پرخاشگری بود که بعد از ضربه علائم به طور قابل توجهی کاهش یافته است. در تست </span><span dir="LTR"><span style="font-size:10.0pt;">MMPI</span></span><span style="font-size:10.0pt;"> تفاوت شخصیتی و بهبودی بیمار نشان داده شد. همچنین بررسی </span><span dir="LTR"><span style="font-size:10.0pt;">MRI</span></span><span style="font-size:10.0pt;"> تغییرات ساختاری در مغز وی را گزارش کرد و در پیگیری 4 ساله نیز مشکل خاصی نداشت. </span></span></p> <p dir="RTL"><span style="font-family:times new roman;"><strong><span dir="RTL"><span style="color:blue;"><span style="font-size:10.0pt;">نتیجه گیری:</span></span></span></strong><span dir="RTL"><span style="font-size:10.0pt;"> این مورد گزارش شده بیمار تابلوی تغییرات شخصیتی و رفتاری پس از ضربه سر را نشان می دهدکه با توجه به نتایج </span></span><span style="font-size:10.0pt;">MRI</span><span dir="RTL"><span style="font-size:10.0pt;"> احتمالا بهبودی ناشی از تخریب ناحیه لوب فرونتال و نیز قسمت قدامی لوب تمپورال (مجاور هیپوکامپ) می باشد، بنابراین اهمیت عوامل نوروفیزیولوژیک در پدید آیی ملاک های تشخیصی را مورد توجه قرار می دهد</span></span></span></p> <p style="text-align: justify;"><span style="font-family:times new roman;"><span style="color:#0000FF;"><strong>BACKGROUND</strong> <strong>AND</strong> <strong>OBJECTIVE</strong><strong>:</strong> </span>Borderline personality disorder which constantly changes the mood, identity and interpersonal relationship is a mental disorder classified in category B. Besides, it leads to behaviors such as drug abuse, impulsive sex and suicide which make more difficult the treatment of these patients. The aim of this study was to report the recovery of a personality problem in a case with borderline personality disorder after the head trauma.</span></p> <p style="text-align: justify;"><span style="font-family:times new roman;"><span style="color:#0000FF;"><strong>CASE</strong> <strong>REPORT:</strong> </span>The personality signs of a 28-year-old boy who had the full criteria of borderline personality disorder including drug abuse, impulsivity, mood instability, periods of anger and aggression were significantly decreased after hitting. Personality difference and improvement of patient were shown in MMPI test and MRI indicated the structural changes in his brain. He had no problem in 4-year-follow up.</span></p> <p style="text-align: justify;"><span style="font-family:times new roman;"><span style="color:#0000FF;"><strong>CONCLUSION:</strong> </span>This report suggested the personality and behavioral changes after head trauma. MRI results indicated that the recovery is probably caused by the destruction area in the frontal lobe and the anterior temporal lobe (near the hippocampus).
