<p dir="RTL" style="text-align: justify;"><span style="font-family:times new roman;"><span style="color:#0000FF;"><strong>سابقه و هدف:</strong></span> ریکتز وابسته به ویتامین<span dir="LTR">D </span> نوع 2(<span dir="LTR">VDDR2</span>= <span dir="LTR"> Vitamin D-</span><span dir="LTR">Dependent Rickets Type 2</span>) بیماری نادر ژنتیکی ناشی از موتاسیون در گیرنده ویتامین <span dir="LTR">D</span> بوده و منجر به مقاومت به اثرات بیولوژیک کلسیتریول خواهد شد. این بیماری معمولا بسته به نوع جهش، حتی به دوزهای بالای کلسیتریول مقاوم بوده و درمان موفق این بیماران مستلزم اصلاح هایپوکلسمی از طریق تجویز کلسیم با دوز بالا و حذف مسیر معیوب رسپتور است. تجویز کلسیم به صورت وریدی علاوه بر نیاز به بستری مکرر و صرف هزینه بالا با عوارض و مشکلاتی همچون آریتمی و سپسیس، عفونت در کاتتر وریدی و هایپرکلسیوری همراه است. هدف از گزارش این موارد، تاثیرات مثبت درمانی کلسیم خوراکی دوز بالا در 4 بیمار مبتلا ریکتز وابسته به ویتامین<span dir="LTR">D</span> نوع 2 می­باشد.</span></p>
<p dir="RTL" style="text-align: justify;"><span style="font-family:times new roman;"><span style="color:#0000FF;"><strong>گزارش موارد:</strong></span> در این مطالعه 4 بیمار مبتلا به <span dir="LTR">VDDR2 </span>که بر اساس علایم بالینی و بیوشیمی ریکتز همراه با آلوپسی تشخیص داده شده اند، در مرکز غدد و متابولیسم کودکان بیمارستان امام رضا (ع) تحت درمان با کلسیم خوراکی با دوز بالا (<span dir="LTR">mg/kg/day</span>300) قرار گرفتند. در مدت کوتاهی بدون ایجاد عوارض جانبی، علاوه بر بهبود یافته های بیوشیمی، افزایش سرعت رشد قدی و قدرت و کشش عضلات مشاهده گردید و حتی یک بیمار در طی یک هفته پس از شروع درمان برای نخستین بار شروع به راه رفتن مستقل نمود. بیماران به مدت یکسال هر 3 ماه تحت پیگیری منظم از لحاظ قد و وزن، سرعت رشد و فاکتور های بیوشیمی شامل کلسیم، فسفر و آلکالن فسفاتاز قرار گرفتند.</span></p>
<p dir="RTL" style="text-align: justify;"><span style="font-family:times new roman;"><span style="color:#0000FF;"><strong><span dir="RTL">نتیجه گیری:</span></strong></span> <span dir="RTL">مناسب به نظر میرسد که در </span><span dir="RTL">هر بیمار</span><span dir="RTL"> مبتلا به </span> VDDR2<span dir="RTL">، بخصوص بیمارانیکه در سن پایین تری تشخیص داده میشوند، بدون در نظر گرفتن نوع موتاسیون در رسپتور ویتامین </span>D<span dir="RTL">،</span><span dir="RTL"> یک تریال 3-6 ماهه کلسیم خوراکی با دوز بالا شروع گردد</span></span></p>
<p style="text-align: justify;"><span style="font-family:times new roman;"><span style="color:#0000FF;"><strong>BACKGROUND AND OBJECTIVE:</strong> </span>Vitamin D-dependent rickets type II (VDDR2) is a rare genetic disorder caused by mutations in vitamin D receptor (VDR) and leads to resistance to biological effects of calcitriol. Based on the type of mutation, this disease is resistant to calcitriol even at high doses of calcitriol and successful treatment of these patients requires hypocalcemic modification through administration of high doses of calcium and bypassing the intestinal defect in VDR signaling. In addition to the need for frequent hospitalization and high costs, intravenous administration of calcium is associated with complications and problems such as arrhythmia and sepsis, venous catheter infection and hypercalciuria. This study aims to report the positive treatment effects of high doses of oral calcium in 4 patients with vitamin D-dependent rickets type II.</span></p>
<p style="text-align: justify;"><span style="font-family:times new roman;"><span style="color:#0000FF;"><strong>CASE REPORT:</strong></span> In this study, 4 patients with vitamin D-dependent rickets type II, diagnosed based on clinical and biochemical symptoms of rickets with alopecia, underwent therapy using high doses of oral calcium (300 mg/kg/day) in pediatric endocrinology and metabolism center of Imam Reza hospital. After a short period, increased growth rate in height, strength and elasticity of muscles was observed in addition to biochemical improvements without serious side effects and even one patient started walking independently within the first week of therapy for the first time. Patients were regularly followed up in terms of height and weight, growth rate and biochemical factors including calcium, phosphorus and alkaline phosphatase every 3 months for one year.</span></p>
<p style="text-align: justify;"><span style="font-family:times new roman;"><span style="color:#0000FF;"><strong>CONCLUSION:</strong></span> Regardless of the type of mutation in vitamin D receptor, it is suggested that a 3-6 months trial of high dose oral calcium be started in each patient with vitamin D-dependent rickets type II, particularly for patients whose disease was diagnosed at lower ages.</span></p>
