تاثیر طول مدت تزریق بر میزان کبودی ناشی از تزریق هپارین زیر جلدی

سابقه و هدف: کبودی از شایع ترین عوارض ناشی از تزریق زیر جلدی هپارین می باشد که موجب محدود کردن ناحیه تزریق، اضطراب، اختلال در تصویر ذهنی از خود، رد درمان توسط بیمار و کاهش اعتماد بیمار نسبت به کارایی پرستار می شود. گرچه روش های گوناگونی جهت به حداقل رساندن این عارضه ناخواسته پیشنهاد شده است ولی متاسفانه تاکنون هیچ یک از این شیوه ها نتوانسته است بطور موثری این عارضه را کاهش دهند. هدف از مطالعه حاضر مقایسه تاثیر طول مدت تزریق بر میزان کبودی ناشی از تزریق هپارین زیر جلدی می باشد.<br>مواد و روشها: این مطالعه نیمه تجربی بر روی 50 بیمار تحت درمان با هپارین زیر جلدی، بستری در بخش های داخلی- جراحی مرکز آموزشی درمانی شهید بهشتی شهر بابل انجام شد. برای هر بیمار دو تزریق، 10 ثانیه ای هپارین زیر جلدی، (شیوه کنترل) و 30 ثانیه ای به (شیوه تجربی)، در عضله دلتوئید دست راست یا چپ، بصورت تصادفی انجام گردید. فاصله دو تزریق 12 ساعت بود و میزان کبودی و اندوراسیون دستها 48 و 60 ساعت بعد از هر تزریق با استفاده از خط کش پلاستیکی قابل انعطاف صورت گرفته و باهم مقایسه شد.<br>یافته ها: سایز کبودی های ایجاد شده ناشی از تزریقات 30 ثانیه ای، به طور معنی داری کمتر از اندازه کبودی های ایجاد شده بدنبال تزریقات 10 ثانیه ای هپارین زیر جلدی بود (001/0=p). میانگین اندازه کبودی در روش تزریق 10 ثانیه ای در 48 و 60 ساعت پس از تزریق (بترتیب 7/19±14و 38/14±26/12) و میانگین اندازه کبودی در روش تزریق 30 ثانیه ای در 48 و 60 ساعت پس از تزریق (بترتیب 55/17±78/9و 98/12±02/8) بوده است. همچنین عدم حضور کبودی، 48 و 60 ساعت بعد از تزریقات 30 ثانیه ای بسیار کمتر از تزریقات 10 ثانیه ای بود (به ترتیب 03/0=p و 001/0=p). مقایسه میزان کبودی بر حسب جنس، در اغلب موارد، میزان کبودی بعد از تزریق 30 ثانیه ای در زنان دریافت کننده هپارین، به طور معنی داری بیشتر از مردان بوده است (001/0=p). همچنین اندازه کبودی در بیماران بخش های داخلی به طور معنی داری بیشتر از بیماران بستری در بخش های جراحی بوده است (001/0=p). بین میزان کبودی و سن بیماران اختلاف معنی داری مشاهده نشد.<br>نتیجه گیری: با توجه به کاهش چشمگیر سایز کبودی ناشی از تزریق هپارین زیر جلدی به دنبال افزایش زمان تزریق، بویژه در زنان و به منظور ارتقاء کیفیت مراقبتی و به حداقل رساندن تجربیات ناخوشایند و استرس زای بیماران، افزایش زمان تزریق هپارین زیر جلدی به 30 ثانیه پیشنهاد می شود.
BACKGROUND AND OBJECTIVE: Bruising is the most common complications that resulting from subcutaneous heparin injection. It causes the limitation of injection area, anxiety, disruption of body image, refusing treatment in patients and the reduction in patients trust in nurse. Although different methods were suggested to lessen these unwanted complications, unfortunately none of them could decrease these complications effectively. The aim of this study was to compare the effect of injection duration on the bruising size of subcutaneous heparin injection.<br>METHODS: This quasi-experimental study was performed on 50 patients who were administered by subcutaneous heparin. They were hospitalized in the internal-surgical ward of Shahid Beheshti hospital in Babol, Iran. For each patient two methods of subcutaneous injection, 10 second injection (control method) and 30 second injection (experimental method) were tried on right and left deltoid muscle accidentally. The interval between two injections was 12 hours. Dimension of hands bruising and indurations were measured and compared using flexible, plastic ruler, 48and 60 hours after each injection.<br>FINDINGS: The size of bruising caused by 30 second injection, was significantly less than 10 second injection (p=0.00). Mean of bruising size in 10 second injection at 48 and 60 hours after injection was14±19.7 and 12.26±14.38, respectively and in 30 second injection at 48 and 60 hours after injection was 9.78±17.55 and 8.02±12.98, respectively. Also, 48 and 60 hours after 30 second injection, absence of bruising, were significantly less than 10 second injection (p=0.03 and p=0.001, respectively). Comparison of bruising size based on sex, females significantly had more bruising than males (p=0.001). Furthermore, patients in internal wards compared to surgical wards had significantly bigger size of bruising (p=0.001). There wasn any significant relationship between age and bruising size.<br>CONCLUSION: The results of this study showed that the increase in injection duration significantly reduced the bruising size. So in order to improve the quality of care and lessen the uncomfortable and stressful experiences of patients it is suggested to increase the duration of subcutaneous heparin injection to 30 seconds.
