فاسیولیازیس در یک کودک مبتلا به آسیت و درمان با تریکلابندازول

سابقه و هدف: فاسیولیازیس یک بیماری دامی است که غالبا گوسفندان را گرفتار می کند. گاهی انسان نیز به عنوان میزبان تصادفی مبتلا می شوند که عمدتا با مصرف گیاهان آلوده به متاسرکرهای فاسیولا هپاتیکا اتفاق می افتد. فرم بالغ انگل در مجاری صفراوی کبد مستقر می شود. اکثر عفونت های انسانی بدون علامت هستند و این مورد یک فرم نادر از این بیماری در یک کودک 8 ساله می باشد. گزارش مورد: کودک 8 ساله ای به علت بزرگی شکم و تب از 4 ماه قبل مراجعه نموده که در معاینه آسیت شدید و هپاتواسپلنومگالی داشته و در بررسی های پاراکلینیکی، ائوزینوفیلی در خون محیطی، گرانولوم ائوزینوفیلیک در کبد و سرولوژی مثبت برای فاسیولا هپاتیکا و تخم این پارازیت در مدفوع مشاهده شد. که بیمار با داروی تریکلابندازول بدون هیچ عارضه ای بهبودی کامل پیدا کرده است. نتیجه گیری: در کودکان در تشخیص افتراقی آسیت همراه با ائوزینوفیلی باید فاسیولیازیس را در نظر داشت و می توان تریکلابندازول را در درمان این بیماری بکار گرفت.
Background and Objective: Fascioliasis is a zoonotic disease that often involves sheep. Sometimes, it may be found in human as an accidental host. It is mainly occurred by consumption of contaminated herbs with metacercarias of fasiola hepatica. The adult phase is in the biliary tree. Most of the human infections are without symptom. This study was done on a case of fascioliasis with uncommon clinical manifestation. Case: An eight-year-old child referred with abdominal distention and with fever from four months ago. Severe ascites and hepatosplenomegaly were found in physical examination. In paraclinical investigations, eosinophilia in peripheral blood smear, eosinophilia granuloma in liver, and positive serologic titers and ova of fasciola hepatica in stool were seen. The patient was successfully treated with triclabendazole without any side effects. Conclusion: In children, fascioliasis should be considered in differential diagnosis of ascites with eosinophilia. The administration of triclabendazole is recommended for children with fascioliasis.
