بررسی اثر دکسترومتورفان بر انتقال عصبی-عضلانی پره پاراسیون عضله دوبطنی گردنی جوجه

سابقه و هدف: دکسترومتورفان یک داروی ضد سرفه (OTC) با سابقه بیش از 45 سال مصرف می باشد. عوارض جانبی آن کم، ولی در دوزهای بالا ایجاد ضعف عضلانی، تضعیف CNS، خواب آلودگی و اختلالات دستگاه گوارش می نماید. در این مطالعه، اثر دکسترومتورفان بر انتقال عصبی-عضلانی، عضله دو بطنی گردنی جوجه بررسی گردید. مواد و روش ها: عضله دو بطنی گردنی پس از جدا سازی از گردن جوجه (با سن سه هفته ای)، در ارگان بس قرار گرفت. جهت حفظ سلامتی عضله و برقراری شرایط فیزیولوژیک، محلول تیرود 37 درجه سانتیگراد به همراه اکسیژن را وارد ظرف یا vessel ارگان بس با حجم 70 میلی متر لیتر نموده و پس از کنترل نهایی با استفاده از دستگاه Stimulator، تحریک الکتریکی با فرکانس 0.1 هرتز، ولتاژ 5 الی 10 ولت و مدت زمان تحریک 0.5 میلی ثانیه ایجاد نمودیم. فعالیت انقباضی بصورت توئیچ (Twitch) از طریق یک ترانس دیوسر به دستگاه پلی گراف منتقل و ثبت گردید. یافته ها: دکسترومتورفان در غلظت های 2.5 الی 5.4 میکرومولار، اثر مهاری بر پاسخ توئیچ به تحریک الکتریکی غیرمستقیم و Contracture استیل کولین اگزوژن داشت. همچنین اثر مهاری دکسترومتورفان توسط فایزوستیگمین و 4-آمینوپیریدین و آنتاگونیزه نشد. افزایش پاسخ توئیچ ناشی از 10 برابر کردن مدت زمان تحریک در حضور دکسترومتورفان تضعیف گردید و تتانوز و تسهیل پس از تتانوز در حضور دکسترومتورفان مهار شد. پاسخ توئیچ به تحریک الکتریکی مستقیم در حضور دکسترومتورفان بطور برجسته مهار شد و مشخص گردید که کافئین توانایی آنتاگونیزه کردن اثر مهاری دکسترومتورفان بر پاسخ توئیچ به تحریک الکتریکی غیرمستقیم را دارد. نمودارهای دوز-رسپانس استیل کولین و کرباکول با کاهش efficacy به سمت راست شیفت پیدا کرد. نتیجه گیری: بنظر می رسد اثر مهاری دکسترومتورفان (5.4 میکرومولار) بر انتقال عصبی-عضلانی، عضله دو بطنی گردنی جوجه، از طریق مهار کانال های کلسیمی مستقر در رتیکولوم سارکوپلاسمیک اعمال شود.
Background and Objective: Dextromethorphan is a well-known over-the-counter antitussive drug with over 45 years of clinical usage experience. Its toxicity is low, but extremely high doses may produce muscle or CNS depression, drowsiness and gastrointestinal upset. In the present study, the effect of dextromethorphan on the neuromuscular transmission of isolated chick biventer cervicis muscle preparation was examined. Methods: This isolated chick (About 3 weeks old) biventer cervicis muscle was put in the organ bath. The organ bath had a vessel with volume of about 70ml, it contained tyrode solution aerated with oxygen and was kept at 37°C. The nerve supplying the twitch-fibers was located in the tendon against which the electrodes were placed. It was stimulated usually at a frequency of 0.1 HZ, duration of 0.5 msec. and voltage of 5-10 volt and twitch response was recorded by a polygraph apparatus after transferring through a transducer. Findings: Dextromethorphan in the range of 2.5 to 5.4 µM has an inhibitory effect on the twitch response to indirect electrical stimuli and contracture of exogenous acetylcholine. The inhibitory effect of dextromethorphan was not antagonized by physostigmine or 4-aminopyridin. Potentiation to twitch response due to 10 fold greater duration of the stimuli was decreased by dextromethorphan. Both tetanosis and facilitating after tetanosis were inhibited in the presence of dextromethorphan. Twitch response to the direct electrical stimuli was inhibited by dextromethorphan. Caffeine significantly antagonized the inhibitory effect of dextromethorphan on the twitch response to indirect electrical stimuli. Dose-response curves of acetylcholine and carbachol in the presence of dextromethorphan shifted to right with a decrease in the efficacy. Conclusion: These results suggested that the inhibitory effect of dextromethorphan on the neuromuscular transmission of the chick biventer cervicis muscle is mediated through blockade of sarcoplasmic reticulum calcium channels.
