Determining the Effective Factors on the Survival Rate and Recurrence of Patients with Endometrial Cancer

Background and Objective: Endometrial cancer is the sixth most common cause of cancer-related death in women. Out of 320,000 women diagnosed with endometrial cancer, 76,000 die every year. Considering the prevalence and mortality of this disease in Iran, we decided to determine the factors affecting the survival of patients and the recurrence rate of this disease in the region.
Methods: In this cohort study, all patients referred to the oncology clinic of Ayatollah Rouhani Hospital and Women's Cancer Clinic during 2017-2018 with endometrial cancer pathology treated with surgery and chemoradiotherapy were included in the study. Demographic and clinical information was recorded in the checklist and the rate of survival, recurrence and survival without recurrence, during the follow-up 1-5 years after surgery was checked by telephone or based on the available information, and at the end, the findings were statistically analyzed and reported.
Findings: The follow-up period was 42.72±32.5 months, the average age was56.63±8.83 years, and 82.3% had no recurrence after surgery. The average overall survival was 122.46 months and it was significant based on the grade (p=0.001).The effect of age on survival was significant (p=0.004), as well as stage and grade; the risk of death in high risk people was 48.6times higher than in low risk people (p=0.004) and most of the death cases (81.2%) occurred in the surgery-radiotherapy group. Compared to chemoradiotherapy, surgery and surgery-radiotherapy methods significantly reduced the risk of death by 98% and more than 80% respectively (p=0.002, p=0.024). Recurrence was significant based on stage and was more in high-risk people (p=0.01) and the majority of people without relapse were low-risk. Most of the relapses in high-risk people were in the abdomen and in low-risk people in the pelvis and vagina.
Conclusion: Stage, grade, type of treatment and age was the factors affecting patient survival and follow-up of patients with endometrial cancer can increase survival and reduce recurrence.
