بررسی شیوع و تعیین مقاومت دارویی و سروتایپینگ استرپتوکوک پنومونیه در ایران

<div style="text-align: justify;"><span style="font-family:Times New Roman;"><strong><span style="color:blue;">سابقه</span></strong> <strong><span style="color:blue;">و هدف: </span></strong><span style="color:black;">استرپتوکوک پنومونیه یکی از عوامل اصلی ایجاد کننده مننژیت، پنومونی، سینوزیت و عفونت گوش میانی می باشد که با مرگ و میر بالایی همراه است. هدف از این مطالعه، مرور کلی و ارائه گزارش توزیع سروتیپ و الگو های مقاومت آنتی بیوتیکی و تعیین فراوانی استرپتوکوک پنومونیه در ایران می باشد</span><span dir="LTR">.</span><span style="font-family:2 Mitra;"></span><br>
<strong><span style="color:blue;">مواد و روش ­ها:</span></strong> در این مطالعه مروری نظاممند، از مقالات منتشره از دی ماه 1368 تا شهریور ماه 1399 در بانک های اطلاعاتی <span dir="LTR">Google Scholar</span>، <span dir="LTR">Scopus</span>، <span dir="LTR">Pubmed</span>،<span dir="LTR">SID </span>، <span dir="LTR">IranMedex</span>، <span dir="LTR">Magiran</span> استفاده شد که بیان کننده توزیع سروتیپی، الگو های مقاومت آنتیبیوتیکی و فراوانی استرپتوکوک پنومونیه در ایران میباشند. واژههای جستجو شامل "استرپتوکوک پنومونیه، مقاومت آنتی بیوتیکی، سروتیپینگ، مرور نظاممند و ایران" می باشند. موارد گزارش مورد، گزارش های صرفاً در خصوص سروگروپ ها و حجم نمونه کمتر از 10 مورد از مطالعه خارج شدند.<span style="font-family:2 Mitra;"></span><br>
<strong><span style="color:blue;">یافته ­ها:</span></strong> از 56 مطالعه، 33 مورد را که دارای معیار ورود بودند، جهت استخراج اطلاعات و آنالیز انتخاب شدند. میانگین شیوع در بیماران و افراد ناقل به ترتیب 12/5% و 20/7% محاسبه گردید. بیشترین میزان مقاومت ایزوله ها نسبت به کوتریموکسازول، پنی سیلین و اریترومایسین بود. شایعترین سروتیپ ها <span dir="LTR">F</span>23 و به ترتیب <span dir="LTR">F</span>19، <span dir="LTR">A/B</span>6، <span dir="LTR">A</span>19 و <span dir="LTR">C</span>18 بودند.<span style="font-family:2 Mitra;"></span><br>
<strong><span dir="RTL"><span style="color:blue;"><span style="font-size:10.0pt;">نتیجه گیری:</span></span></span></strong> <span dir="RTL"><span style="font-size:10.0pt;">بر اساس نتایج این مطالعه استفاده نامناسب از آنتیبیوتیکها و متعاقب آن گسترش ایزوله های مقاوم پنوموکوک در کشورمان نگران کننده میباشد</span></span><span dir="RTL"><span style="font-size:10.0pt;">.</span></span></span></div>
<div style="text-align: justify;"><span style="font-family:Times New Roman;"><strong>BACKGROUND AND OBJECTIVE:</strong> Streptococcus pneumoniae is one of the main etiologies of meningitis, pneumonia, sinusitis, and middle ear infection associated with significant morbidity and mortality. This report aims to review and report the serotype distribution and antimicrobial resistance patterns and determine the frequency of S. pneumoniae in Iran.<br>
<strong>METHODS: </strong>A systematic literature review of the literature published from January 1990 to August 2020 was performed to identify articles that have been published in Google Scholar, Scopus, PubMed, SID, IranMedex, and Magiran databases that describe the serotype distribution, antimicrobial resistance patterns and frequency of S. pneumoniae in Iran. The search terms were "Streptococcus pneumoniae, Antibiotic resistance, Serotyping, Systematic review, and Iran". The exclusion criteria were review articles or case reports, reports only emphasizing on serogroups rather than serotypes, and having isolates fewer than 10.<br>
<strong>FINDINGS:</strong> Of 56 reports, 33 publications that met our inclusion criteria were selected for data extraction and analysis. The frequency of S. pneumoniae in patients and carriers was estimated at 1.5% and 20%, respectively. Highest levels of resistance were against co-trimoxazole followed by penicillin, and erythromycin. The most common serotypes were 23F followed by 19F, 6A/B, 19A, and 18C.<br>
<strong>CONCLUSION:</strong> Based on the results of this study, the inappropriate use of antibiotics and the subsequent spread of resistant pneumococcal isolates in our country is worrisome.</span></div>
