fa ارزش تشخیصی آسپیراسیون سوزنی ظریف (‏FNA‏) در مقایسه با نتایج پاتولوژی در تشخیص ‏ندول‌های تیروئید در بیمارستان شهید بهشتی بابل، شمال ایران، طی 22 سال The Diagnostic Value of Fine Needle Aspiration as Compared to ‎Pathology Results in Diagnosis of Thyroid Nodules: A 22-Year ‎Follow-up Study جراحی surgery مقطعی Cross Sectional سابقه و هدف: آسپیراسیون سوزنی ظریف (FNA=Fine Needle Aspiration) تیروئید به عنوان یک روش انتخابی در بررسی ندول‌های تیروئید می باشد که اخیراً نقش آن در مدیریت ندول‌ها مورد شک قرار گرفته است. این مطالعه با هدف مقایسه ارزش تشخیصی آسپیراسیون سوزنی ظریف با نتیجه پاتولوژی در بررسی ندول‌های تیروئید در بیمارستان شهید بهشتی بابل، شمال ایران طی 22 سال انجام گردیده است.<o:p /> مواد و روشها: این مطالعه‌ مقطعی بر روی نمونه سیتولوژی 225 بیمار که طی 22 سال (91-1369)‌ بیمارستان شهید بهشتی شهرستان بابل تحت عمل تیروئیدکتومی قرار گرفتند، انجام شد. سن، جنس، پزشک انجام دهنده FNA، نوع و سمت جراحی، نتایج FNA و پاتولوژی ثبت گردیدند. نتایج FNA بر اساس گزارش پاتولوژی به چهار گروه ناکافی، خوش خیم، بدخیم و مشکوک طبقه‌بندی و نتیجه پاتولوژی نیز به دو گروه خوش خیم و بدخیم تقسیم شده و مورد مقایسه قرار گرفتند.<o:p /> یافتهها: FNA در تشخیص ندول‌های بدخیم تیروئید، حساسیت 60 درصد، ویژگی 96 درصد، ارزش اخباری مثبت 65 درصد، ارزش اخباری منفی 95 درصد و دقت تشخیصی 81/8 درصد داشته است. از 225 نمونه سیتولوژی، 127 مورد خوش‌خیم (56/4%)، 47 مورد مشکوک (20/9%)، 27 مورد بدخیم (12%) و 24 مورد ناکافی (10/7%) گزارش شد. همچنین تشخیص پاتولوژی نهایی در بیماران مورد مطالعه در 150 بیمار (66/7%) خوش‌خیم و در 75 بیمار (33/3%) بدخیم بود.<o:p /> نتیجه گیری: براساس نتایج این مطالعه از آسپیراسیون ظریف سوزنی نیز می توان در تشخیص ندول های تیروئید استفاده کرد.<o:p />   BACKGROUND AND OBJECTIVE: Recently, the efficiency of thyroid fine needle aspiration (FNA) as a method of choice in evaluation and management of thyroid nodules has been questioned. The aim of this study was to determine the diagnostic value of FNA as compared to pathology results in diagnosis of thyroid nodules in Shahid Behashti Hospital of Babol, Iran, within 22 years. <o:p /> METHODS: This cross-sectional study was performed on cytology samples of 225 patients who were undergoing thyroidectomy at Shahid Beheshti Hospital of Babol during 1990-2012. Patients’ age and gender, physician's experience in performing FNA, type and location of surgery, FNA samples and pathology results were recorded. According to the pathology reports, FNA results were divided into four groups i.e. inadequate, benign, malignant and suspicious. The pathology results were divided into benign and malignant groups. The results of both methods were compared with each other.<o:p /> FINDINGS: The sensitivity and specificity of FNA in diagnosis of thyroid nodules were 60% and 96%, respectively. Also, its positive and negative predictive values and diagnostic accuracy were 65%, 95% and 81.8%, respectively. Among the 225 cytology samples, 127 (56.7%), 47 (20.9%), 27 (12%) and 24 (10.7%) were benign, suspicious, malignant, and inadequate, respectively.
