بررسی نتایج هیستروسالپنگوگرافی در بیماران نابارور مراجعه کننده به یک کلینیک خصوصی رادیولوژی سال (1386-1377)

سابقه و هدف: هیستروسالپنگوگرافی روش مهمی در تشخیص علت آناتومیک ناباروری به شمار می رود که با توجه به مقرون به صرفه بودن بعنوان تست غربالگری مورد استفاده قرار می گیرد، لذا این مطالعه با هدف بررسی میزان شیوع و چگونگی اختلالات آناتومیک بیماران به روش هیستروسالپنگوگرافی انجام شد.<br>مواد و روشها: این مطالعه توصیفی طی سالهای 86-1377 برروی 1492 زن نابارور که جهت بررسی علت نازایی و رد علل آناتومیک توسط متخصصین زنان معاینه به کلینیک رادیولوژی ارجاع شده بودند، انجام گرفت. بیماران تحت هیستروسالژنگوگرافی در روز 12-6 سیکل خود قرار گرفتند و سپس نتایج حاصله ثبت و مورد بررسی قرار گرفت.<br>یافته ها: از 1492بیمار، (58%) 865 مورد نازایی اولیه و (42%) 627 مورد نازایی ثانویه داشتند. نتایج هسیتروسالپنگوگرافی در(60% ) 895 مورد کاملاً طبیعی بوده و در (40%) 597 مورد یک یافته غیر طبیعی وجود داشت که (5/61% ) 367 مورد یافته قابل گزارش در رحم، (32%) 191 مورد در لوله رحمی راست، (29%) 179 مورد در لوله رحمی چپ، (18%) 108 مورد چسبندگی پریتوئن یا اطراف لوله و(16%) 96 مورد در سرویکس داشتند. عوامل عضوی نازایی در (7/27%) 413 مورد مشاهده شد که شامل (8/17%) 265 مورد گرفتاری لوله های رحمی، (8%) 119 مورد چسبندگی پریتوئن یا اطراف لوله و (7/2%) 40 مورد چسبندگی کاویته رحمی بوده است.<br>نتیجه گیری: نتایج این مطالعه نشان داد که عوامل آناتومیک نقش زیادی در ایجاد ناباروری ندارند و احتمالا عوامل غیر ارگانیک به عنوان عوامل شایعتر نازایی مطرح می باشند و همچنین فقط درصدی از چسبندگی های پریتوئن و اطراف لوله توسط هیستروسالپنگوگرافی قابل تشخیص هستند که پیشنهاد می شود مطالعاتی بر پایه لاپاراسکوپی نیز انجام شود.
BACKGROUND AND OBJECTIVE: Hysterosalpingography (HSG) is an important diagnostic and screening tool in the evaluation of uterine cavity and fallopian tubes in infertile women. This study was performed to evaluate the prevalence and distribution of anatomical abnormalities in infertile women.<br>METHODS: In this descriptive study, 1492 infertile women were enrolled in this study from 1999-2007 to evaluate the probable anatomical abnormalities as the etiology of their infertility. The patients were referred by the gynecologist and the procedure was done on the 6th to 12th day of their menstrual cycles. Obtained results were recorded and evaluated.<br>FINDINGS: There were 1492 patients who eligible for evaluation, including 865 (58%) primary infertile and 627 (42%) secondary infertile women. The results of HSG were totally normal in 895 (60%) and the others had one or more positive reporting findings including 367 (61.5%) in the uterus, 191 (32%) in the right tube, 179 (29%) in the left tube, 108 (18%) in the peritoneal or peritubal region and 96 (16%) in the cervix. Organic pathologies which could be directly influences fertility were showed in 413 (27.7%) of the studying population including 265 (17.8%) involvement of uterine tubes, 119 (8%) peritoneal or peritubal adhesions and 40(2.7%) uterine cavity adhesions.<br>CONCLUSION: According to the results of this study the anatomic factors are not significant as the reason of infertility in our patients and nonorganic factors are probably more common. Besides just a few peritoneal and peritubal adhesions can be diagnosed by HSG which need to be confirmed by laparoscopy.
