مقایسه اثرات کلوزاپین، اولانزاپین و ریسپریدون با هالوپریدول بر سطح خون ناشتا، کلسترول و تری گلیسرید در مبتلایان به اختلالات سایکوتیک

سابقه و هدف: اختلالات سایکوتیک مهمترین و شایعترین اختلالات روانپزشکی می باشد و داروهای آنتی سایکوتیک (ضد روانپریشی) آتیپیک، به علت نداشتن عوارض خارج هرمی مورد توجه پزشکان و بیماران می باشند، اما از آنجائیکه گزارشهایی مبنی بر وقوع اختلالات متابولیک در بیماران مصرف کننده این داروها منتشر شده. لذا این مطالعه به منظور مقایسه اثر داروهای آنتی سایکوتیک آتیپیک با هالوپریدول انجام شد.<br>مواد و روشها: این مطالعه کوهورت بر روی 75 نفر از بیماران مبتلا به یکی از اختلالات سایکوتیک(براساس معیارهای DSM-IV) انجام شد. بیماران با توجه به پاسخ درمانی قبلی و یا شروع جدید سایکوز در یکی از چهار گروه درمانی کلوزاپین، اولانزاپین، ریسپریدون (گروه آتیپیک) و هالوپریدول(گروه تیپیک) قرار گرفتند. بیماران مقاوم به درمان در گروه کلوزاپین و بیماران مبتلا به اختلال دو قطبی در گروه الانزاپین قرارگرفتند. سطح سرمی قندخون ناشتا، کلسترول و تری گلیسیرید بیماران قبل از شروع درمان و 6 هفته بعد از آن اندازه گیری و با هم مقایسه شد.<br>یافته ها: از 75 بیمار بستری یا سرپایی 45 نفر از آنها در گروه آتیپیک و 30 نفر در گروه هالوپریدول قرار گرفتند. دو گروه از نظر سن و جنس همسان بودند. قند خون ناشتا، میانگین سطح قندخون ناشتا، تری گلیسرید و کلسترول در گروه درمانی داروهای آتیپیک قبل از درمان به ترتیب 2/16±7/90، 5/35±144 و 1/25±5/163 میلی گرم بر دسی لیتر بود و در گروه هالوپریدول به ترتیب 27±6/88، 5/38±5/162 و 23±2/164 میلی گرم بر دسی لیتر بود که بعد از درمان در گروه درمانی داروهای آتیپیک به ترتیب 1/13±4/94، 8/50±168 و 5/33±6/168 میلی گرم و دسی لیتر در گروه هالوپریدول بعد از درمان به ترتیب 27±9/92، 9/45±3/175 و 5/26±164 میلی گرم بر دسی لیتر شد که اختلاف معنی داری باهم نداشتند.<br>نتیجه گیری: با توجه به نتایج این مطالعه که داروهای آتیپیک و تیپیک تفاوت قابل ملاحظه ای بر تغییرات سطح خون ناشتا، تری گلیسرید و کلسترول نداشتند. می توان از هر یک از این داروها در شرایط خاص بیمار استفاه نمود.
BACKGROUND AND OBJECTIVE: Atypical antipsychotic drugs are first line treatment for psychotic disorders and well tolerated by the patients because of less extra pyramidal side effects and better efficacies. But there are different reports about metabolic dysfunctions that are associated with use of these drugs. So the aim of this study was to compare these side effects among atypical antipsychotic drugs (clozapine, olanzapine, risperidone) and haloperidol.<br>METHODS In this cohort study psychotic patients (according to DSM IV criteria) due to their response to previous treatment regiment or new onset of disorder were received clozapine, olanzapine, risperidone (atypical group) and haloperidol (typical group). Patients who were resistant to other drugs treated with clozapine and bipolar patients received olanzapine. Fasting blood level of glucose, cholesterol and triglyceride of all patients were measured before initiating and six weeks after drug administration. <br>FINDINGS: 75 inpatients or out patients with variety of psychotic disorders enrolled the study, as 45 patients were in atypical and 30 in typical drug groups. Two groups were matched by age and sex. The mean level of fasting blood glucose, triglyceride and cholesterol of patients in atypical drug group before treatment was 90.7%±16.2, 144±35.5 and 163±25.1 mg/dl, respectively and in typical group it was 88.6±27, 162.3±38.5 and 164.2±25 mg/dl, respectively. The mean level of fasting blood glucose, triglyceride and cholesterol of patients in atypical drug group after treatment was 94.4±13.1, 168±50.8 and 168.6±33.5 mg/dl, respectively and in typical group was 92.9±27, 175.3±45.9 and 164±26.5 mg/dl, respectively that showed no significant difference. <br>CONCLUSION: Antipsychotic drugs (either typical or atypical) didn’t increase fasting blood level of glucose, cholesterol and triglyceride in psychotic patients.<br>
