مقایسه تاثیر دو روش انسولین درمانی بر عارضه ماکروزومی جنین در زنان دیابتیک باردار

سابقه و هدف: دیابت بارداری اثرات سو از قبیل افزایش شیوع پره اکلامپسی، عفونت بارداری، ناهنجاری مادرزادی، مرگ داخل رحمی، دیسترس تنفسی، هیپوگلیسمی بدنبال دارد. از عوارض مهم آن افزایش وزن جنین بالای 4000 گرم (ماکروزومی) است. کنترل مناسب قند خون می تواند بسیاری از عوارض دیابت از جمله ماکروزومی را کاهش دهد. یکی از مشکلات در مامایی روش انسولین درمانی مناسب جهت تنظیم قند خون است که در این بررسی دو روش انسولین درمانی از نظر تاثیر عارضه ماکروزومی جنین مقایسه می شوند تا روش مناسب توصیه گردد. مواد و روش ها: جمعیت مورد مطالعه 200 زن باردار زیر 32 هفته با قند خون ناشتای بالای 105mg/dl بودند که به دو گروه تقسیم شده، از طریق دو روش انسولین درمانی قند خون تنظیم گردید. در روش (1) اندازه گیری قند خون 6 بار در روز انجام شده که با چهار الگوی تجویز انسولین به اجرا گذاشته شد، در روش (2) اندازه گیری قند خون 3 بار روزانه و استفاده از تجویز انسولین بصورت دو سوم انسولین NPH و یک سوم انسولین کریستال بود. پس از تنظیم قند خون در هر دو گروه و تکمیل پرسشنامه تا انتهای بارداری بیمار تحت نظر قرار داشت و بعد از زایمان اطلاعات مربوط به نوزاد تکمیل گردید. یافته ها: میزان ماکروزومی و میانگین وزن در گروه (1) به ترتیب 15.3 درصد، (588±3538 گرم) و در گروه (2) 17.2 درصد، (467±3563) گرم بود که از این نظر بین دو گروه اختلاف آماری معنی داری وجود نداشت. از نظر عوارض دیابت بر مادر و جنین و نوزاد اختلاف معنی داری در گروه مشاهده نشد. در مورد میزان سزارین نیز در دو گروه اختلاف واضحی دیده نشد. نتیجه گیری: بنابر یافته ها دو روش انسولین درمانی (1) و (2) از نظر کنترل عوارض دیابت بخصوص ماکروزومی یکسان بوده ولی روش (2) به دلیل اینکه اندازه گیری قند خون روزانه در آن 3 بار کمتر از روش (1) است، پذیرش بهتری دارد.
Objective: One of the major side effects of maternal diabetes is that the fetus weights to be more than 4000gr named as macrosomia. A suitable control of the blood sugar can strongly decrease the diabetes complications, such as macrosomia. We tried to compare two insulin therapy methods with regard to macrosomic effects on the fetus in order to suggest the most practical and suitable one. Methods: The subjects were 200 pregnant women below 32 weeks of pregnancy with a blood sugar level over 105 mg/dl before having breakfast. They were divided into two groups, each of which subjected to one of the insulin regulation methods. In the first method, the blood sugar level was measured six times a day, being followed by four different patterns of insulin prescription. In the second method, blood sugar level was measured three times a day, and the prescribed insulin was an integration of 2/3 NPH insulin and 1/3 crystal insulin. Data were collected with using questionnaire before and after labor. Findings: The rate of macrosomia and the average weight in the first group was 7.6%, (3538±588 g) and in the second group was 8.6%, (3563±467 g). There was no meaningful difference between the two groups in terms of the average weight, and other effects of diabetes on mother, fetus, infant and cesarean rate. Conclusion: Therefore, these methods of insulin therapy, in terms of the special macrosomia diabetic effects of diabetes are the same, but in second method, however, is much more advantageous since measuring of the blood sugar level is three rimes less than other one.
