مقایسه سرولوژی (ELISA) با آندوسکوپی در پیگیری بیماران H.pylori درمان شده
A comparison between serology (ELISA) and endoscopy and biopsy in the follow up of patients after treatment of H.pylori infection

سابقه و هدف: در حال حاضر ریشه کن کردن H.pylori در اولسر معده، دوازدهه و لنفوم نوع MALT توصیه می شود. برای پیگیری بیمارانی که تحت درمان آنتی بیوتیکی قرار می گیرند می توان از روش های تهاجمی و غیر تهاجمی استفاده نمود. این مطالعه بمنظور مقایسه روش تهاجمی (آندوسکوپی+بیوپسی) و روش اندازه گیری آنتی بادی های ضد H.pylori صورت گرفته است. مواد و روش ها: این مطالعه مقایسه ای بر روی 150 بیمار که به علل مختلف جهت آندوسکوپی قسمت فوقانی گوارش مراجعه کردند، انجام شد. افرادی که پس از بررسی پاتولوژیکی و بیوپسی معده، H.pylori در آنها مثبت بود انتخاب و به صورت اتفاقی در سه گروه 50 نفری تحت درمان چهار دارویی (امپرازول+بیسموت+مترونیدازول+آموکسی سیلین) قرار گرفتند. یک ماه بعد جهت گروه اول آندوسکوپی+بیوپسی، گروه دوم تست سرولوژی H.pylori ELISA-IgA و گروه سوم تست سرولوژی H.pylori ELISA-IgG درخواست شد تا میزان ریشه کن شدن H.pylori بررسی و مقایسه گردد. 23 بیمار به علل مختلف از مطالعه خارج شدند و در گروه اول 41 بیمار و در گروه دوم 42 بیمار و در گروه سوم 44 بیمار بررسی گردیدند. یافته ها: در گروه اول که آندوسکوپی مجدد+بیوپسی انجام شد H.pylori در 70.7% منفی، در گروه دوم در 35.7 درصد (P<0.001) و در گروه سوم در 20.5% منفی بود (P<0.001) که این اختلاف از نظر آماری معنی دار بود. نتیجه گیری: در پیگیری کوتاه مدت (یک ماه) چون کاهش آنتی بادی ها از حساسیت کافی برخوردار نمی باشند علیرغم آنکه آندوسکوپی مجدد+بیوپسی یک عمل تهاجمی نسبت به سرولوژی می باشد، توصیه می گردد.
Objective: Helicobacter pylori eradication is recommended in gastric and duodenal ulcer and MALT lymphoma management. For following up for patients after treatment of h.pylori infection, invasive and non-invasive procedures can be performed. The objective of this study was to compare invasive and non-invasive procedures. Methods: 150 patients with h.pylori positive were selected for our study and divided into three groups. In all groups quadruple therapy (Omeprazole, bismuth, metronidazole and amoxicillin) was done. One month later endoscopy and biopsy, serology (H.pylori ELISA IgA) and serology (H.pylori ELISA IgG) were done in first, second and third group respectively and compared for h.pylori eradication. Findings: 127 patients remained as subjects in our study. In first group, endoscopy and biopsy was done again and we found no h.pylori in 70.7% of the cases of the first group, 35.7% and 20.5% of the cases of the second and third group respectively and this difference was statistically meaningful (P<0.001). Conclusion: In spite of the fact that endoscopy and biopsy in comparison to serology is an invasive procedure, it is recommended for short term follow up of patients (One months).
