رابطه علائم نورولوژیک ظریف با اسکیزوفرنی

سابقه و هدف: بیماری اسکیزوفرنی از شایعترین بیماری ها در حیطه روانپزشکی است که اخیرا تلاش های زیادی برای یافتن علل بیولوژیک و ارگانیک این بیماری صورت گرفته است. یکی از مواردی که بیشتر مورد توجه قرار گرفته است علائم نورولوژیک ظریف و ارتباط آن با بیماری اسکیزوفرنی است. این مطالعه به منظور مقایسه علائم نورولوژیک ظریف بین بیماران اسکیزوفرن و خواهر یا برادر آنها و گروه شاهد انجام شده است. مواد و روش ها: جمعیت مورد مطالعه شامل 30 بیمار اسکیزوفرن، 30 نفر خواهر یا برادر این بیماران و 30 نفر از کارکنان بیمارستان زارع ساری، به عنوان گروه شاهد می باشند. وجود علائم نورولوژیک ظریف در آنها مورد ارزیابی قرار گرفت. سپس یک نمره کلی از نظر این علایم برای هر آزمودنی بدست می آید، در نهایت این گروه ها با هم مقایسه می شوند. یافته ها: میانگین علائم نورولوژیک در بیماران اسکیزوفرن بیشتر از خواهر یا برادر این بیماران و گروه شاهد است. همچنین میانگین نمرات علائم نورولوژیک خواهر یا برادران بیماران اسکیزوفرن بیشتر از گروه شاهد است. بعلاوه بین نمره کلی علائم نورولوژیک ظریف و سابقه فامیلی بیماری اسکیزوفرنی در گروه بیماران اسکیزوفرن ارتباط معنی داری وجود داشت (P=0.034). یعنی بیماران اسکیزوفرنی که سابقه فامیلی از نظر بیماری اسکیزوفرنی داشتند، نمره کلی از نظر علائم نورولوژیک ظریف بیشتر نسبت به سایر بیماران داشتند. هیچ ارتباطی بین سن و جنس و میانگین نمره کلی این علائم وجود نداشت. نتیجه گیری: با توجه به یافته های این مطالعه که علائم نورولوژیک ظریف در بیماران اسکیزوفرن از دو گروه دیگر بیشتر است، دیدگاه بیولوژیک و ارگانیک در مورد اتیولوژی اسکیزوفرنی را تقویت می کند، این علائم می توانند به عنوان یک مارکر بیولوژیک قوی در بیماری اسکیزوفرنی مطرح شوند و با عنایت به شروع زودرس درمان با آنتی سایکوتیک ها، این علائم می توانند در سرنوشت بیماری تاثیر داشته باشند.
Background and Objective: Schizophrenia is one of the most prevalent diseases in psychiatry. A lot of efforts have been made to find out the biologic and organic causes of this disease. One of the cases, which more attract the attention of researchers, is soft neurologic signs and their relationship with Schizophrenia. The objective of this study was to compare the prevalence of soft neurologic signs between Schizophrenia patients and their sisters and brothers, and normal group. Methods: 30 Schizophrenia patients, 30 persons of their sisters and brothers, and 30 persons of normal group were chosen and evaluated in view of soft neurologic signs. Then a total score was obtained in view of signs for each case. Finally these groups were compared with each other. Findings: The average of soft neurologic signs in Schizophrenia patients was more than in their sisters and brothers and in normal group. The average of scores of soft neurologic signs in these patients’ sisters and brothers was also more than in the normal group. There was also a significant relationship between the total score of the soft neurologic signs and the family history of Schizophrenia in Schizophrenia patients (P=0.034), it means that the patients having a family history for Schizophrenia, had a higher total score than other patients in view of soft neurologic signs. There was no relationship between age, sex and the average of total score of these signs. Conclusion: This findings that neurologic signs in Schizophrenia patients were more than in the 2 other groups confirms the biologic and organic view about the etiology of Schizophrenia, these signs can be raised as a strong biologic marker of Schizophrenia. Considering the early beginning of treatment by anti-psychotic drugs, these signs can influence the outcome of this disease.

گزارش دو مورد مایستومای پا در کرمان و بررسی موارد گزارش شده قبلی در ایران
Report of two cases with Mycetoma of foot in Kerman and overview of other reported cases from Iran

سابقه و هدف: مایستوما بیماری موضعی، مزمن و زیر جلدی است که غلاف عضلات و استخوان ها را مبتلا می نماید و با راه های خروجی سینوسی شکل متعدد به پوست مشخص می شود. بیش از 20 گونه قارچ یا باکتری در اتیولوژی مایستوما دخیل شناخته شده است. دو مورد از این بیماری که به درمان دارویی تنها یا توام با جراحی پاسخ مناسب داده اند معرفی می گردند. گزارش مورد: بیمار اول، مردی 41 ساله می باشد که از سن 6 سالگی ضایعه ای بدون درد و متورم در پشت پای راست بیمار مشاهده شد. بعد از چند ماه که ضایعه بزرگتر شد و چرک از مجاری آن خارج می گردید اقدام به جراحی شد. پس از 10 سال بدنبال برخورد با پلیسه آهن، تورم و درد پا مجددا شروع شد که آزمایشات و رادیوگرافی مایستومای پا را تایید نمودند که به درمان با کوتریموکسازول و آمیکاسین جواب داده و تا 1.5 سال بعد بیماری برگشت نداشت. بیمار دوم مردی 38 ساله می باشد که با درد و تورم غیر گوده گذار پا متعاقب ضربه مراجعه کرد و با تشخیص آرتریت عفونی تحت درمان قرار گرفت و بدلیل عدم بهبودی پس از شش ماه و با انجام رادیوگرافی و آزمایشات لازم در این بیمار نیز مایستومای پا مسجل گردید که درمان با کوتریموکسازول برای بیمار شروع شد و تا یک سال بعد نیز بیماری برگشت نداشت. نتیجه گیری: با توجه به اینکه بیماری با بهداشت عمومی مرتبط بوده و سیر معلولیت زایی بیماری طول کشنده می باشد تاکید بر رعایت موازین بهداشتی توسط افراد و کشش برای تشخیص زودرس بیماری اهمیت دارد.
Background and Objective: Mycetoma is a chronic progressive and destructive subcutaneous disease that can involve fascia and bone. More than 20 microbial and fungal agents can cause Mycetoma. Cases: The first case was a 41-year-old man that a swollen and painless lesion was observed in dorsum of his right foot when he was six. After a few months that the lesion got larger and pus to come out of its ducts, operation was done. After 10 years, he had pain and swelling in his foot again because of touching with iron foreign body. Mycetoma of foot was confirmed by radiology and testing that treated with co-trimoxazole and amikacin and a recurrence of the disease was not observed for 1.5 years. The second case was a 38-year-old man who referred with pain and swelling of foot after a hit that with diagnosis of infectious arthritis was treated. After 6 months due to lack of improvement, Mycetoma of foot was confirmed by testing and radiology which treated with co-trimoxazole and the recurrence of the disease was not seen until one year later. Conclusion: Low environmental and public hygiene due to this disorder with long-term morbidity. According to this report, considering health care by individuals and early diagnosis is important.
