en بررسی تغییرات لب فوقانی به دنبال برش عضله پایین ‌برنده تیغه بینی در جراحی زیبایی بینی An Evaluation of Changes in the Upper Lip Following Depressor Septi Nasi Muscle Resection in Rhinoplasty جراحی فک و صورت Oral and Maxillofacial Surgery توصیفی Dicscriptive <div style="text-align: justify;">سابقه و هدف: توانایی پیش‌ بینی تاثیر روش‌های مختلف جراحی بینی بر تغییرات ایجاد شده در اجزای صورت حائز اهمیت است. هدف از این مطالعه پیش ‌بینی تغییرات احتمالی لب فوقانی به دنبال برش عضله پایین ‌برنده تیغه بینی به منظور انتخاب مناسب در جراحی زیبایی بینی می‌باشد. مواد و روش‌‌ها: در این مطالعه مقطعی، 22 بیمار کاندید جراحی زیبایی بینی که در سال 1401 در بیمارستان‌ها و کلینیک‌های خصوصی شهر بابل تحت عمل قرار گرفتند، از نظر تغییرات لب فوقانی به دنبال برش عضله پایین ‌برنده تیغه بینی بررسی شدند. اندازه طول لب بالا (از ناحیه کلوملا تا لبه فوقانی ورمیلیون لب بالا)، نمایش ورمیلیون (از لبه بالایی ورمیلیون تا انتهای تحتانی لب بالا) و دندان‌های قدامی (لبه تحتانی ورمیلیون تا لبه تحتانی ثنایایی دندان‌های ثنایای میانی فک فوقانی) و نمایش لثه (از لبه تحتانی ورمیلیون لب بالا تا gingival zenith دندان‌های ثنایای میانی فک فوقانی) بین فواصل زمانی قبل از جراحی، 14 روز، 1 و 3 ماه پس از جراحی با اندازه‌گیری‌های مکرر بررسی و مقایسه شدند. یافته‌ها: میانگین طول لب بالا قبل از جراحی 3/05±17/82 میلی‌متر که به ‌طور معنی‌داری 3 ماه بعد از جراحی به 3/22±20/77 میلی‌متر افزایش یافت (0/001>p). میانگین نمایش ورمیلیون نیز به طور معنی‌داری از 2/12±6/50 قبل از جراحی به 2/30±7/44 میلی‌متر 3 ماه بعد از جراحی افزایش یافت (0/001>p). متوسط نمایش دندان‌های قدامی 3 ماه پس از جراحی 1/13±2/14 بود که نسبت به قبل از جراحی 1/43±2/58 میلی‌متر کاهش معنی‌داری داشت (0/001>p). نتیجه‌گیری: نتایج مطالعه نشان داد که برش عضله پایین ‌برنده تیغه بینی موجب افزایش طول لب بالا و نمایش ورمیلیون و کاهش نمایش دندان‌های قدامی در حالت استراحت و لثه‌ها در حالت لبخند می‌شود. بنابراین تحلیل لبخند برای هر فرد قبل از انجام جراحی زیبایی بینی توصیه می‌شود. </div> <div style="text-align: justify;">Background and Objective: The ability to predict the effect of different rhinoplasty techniques on changes in facial features is important. The present study was conducted to predict possible changes in the upper lip following depressor septi nasi muscle resection in order to make appropriate choices in rhinoplasty. Methods: In this cross-sectional study, 22 rhinoplasty candidates who underwent surgery in hospitals and private clinics in Babol in 2022 were examined regarding changes in the upper lip after depressor septi nasi muscle resection. The length of the upper lip (from the columella to the upper edge of the vermilion of the upper lip), the vermilion view (from the upper edge of the vermilion to the lower end of the upper lip), and the anterior teeth (the lower edge of the vermilion to the lower incisor edge of the maxillary central incisors), and the gingival view (from the lower edge of the vermilion of the upper lip to the gingival zenith of the maxillary central incisors) were examined and compared between the time intervals before surgery, 14 days, 1, and 3 months after surgery using repeated measurements. Findings: The mean length of the upper lip was 17.82±3.05 mm before surgery, which significantly increased to 20.77±3.22 mm 3 months after surgery (p<0.001). The mean vermilion view also significantly increased from 6.50±2.12 mm before surgery to 7.44±2.30 mm 3 months after surgery (p<0.001). The mean anterior teeth projection 3 months after surgery was 2.14±1.13 mm, which was a significant decrease compared to 2.58±1.43 mm before surgery (p<0.001). Conclusion: The results of the study showed that depressor septi nasi muscle resection increases the length of the upper lip and the view of vermilion, and reduces the view of the anterior teeth at rest and the gingiva when smiling.
