Evaluation of flowcytometry assay in identifying platelet resistance in AML patients

Aim: The aim of this study was to evaluate the efficacy of flow cytometry in predicting the results of platelet transfusion, platelet resistance, and selecting platelets compatible with patients.
Materials and Methods: In this case control study, 15 patients with AML who had history of platelet transfusion for more than two times were included. Following the labeling of platelets with CMFDA and patients serum, the amount of phagocytosis of platelets by monocytes was investigated.
Results: The mean and standard deviation of the results of the potency of phagocytosis of platelets by monocyte in the control group was 18.27 ± 2.86%, in the patient with Immune platelet resistance was 68.47 ± 10.40%, and in the patients with non-Immune platelet resistance was 36.33 ± 15.19. The results show that there is a significant difference between the mean platelet phagocytosis of monocyte in both patient and control groups (p = 0.001).
Also, there was a significant negative correlation between platelet phagocyte capacity by monocytes and CCI for one hour and 24 hours separately (p = 0.001).
Conclusion: as flowcytometry is a sensitive method, using CMFDA can be useful in evaluation of platelets, , so it can be use for platelet cross match by flowcytometric methods to achieve better results in detecting patients with platelet resistance.
